A hack is a common term within the ROM Editing/Translating Community. It defines a project that one does by editing an already existing game to modify and/or include new features. In the Sonic Community, most hacks are edits of the Genesis/Megadrive Sonic the Hedgehog games.
There are two types of hacks: Hexadecimal hacks which are projects that only required Hexadecimal editing inside the ROM; and Assembly hacks which are made from a disassembly so that the hacker can modify and add things down to the core.
